The death of 28-year-old Jefferies associate Carter McIntosh in January was down to the “toxic effects” of fentanyl and cocaine, a Dallas medical examiner report has concluded.

McIntosh, whose death prompted fresh outrage from junior bankers about the industry’s long hours culture, died from accidental causes, according to the report released on 7 May.
